ONE visitor per patient may be permitted to enter facility under one or more of the following circumstances:
No other visitors are permitted to enter facility. Patients and permitted visitors must wear a mask or other face covering over his or her nose and mouth for the duration of their visit while inside the facility if medically tolerable.
Every patient and permitted visitor must be screened prior to entry to facility.

Patients who have symptoms or other COVID-19 risk factors may be asked to reschedule their appointment. For the health and safety of patients, visitors, and our healthcare staff, we reserve the right to deny visitation, reschedule patient appointments, or make alternative treatment arrangements at any time. Thank you for helping us provide a safer environment for our patients and staff.
If COVID test is positive, then quarantine for at least 10 days since onset of symptoms AND at least 24 hours of improvement in symptoms.If COVID test is negative, then quarantine for 14 days from last known COVID exposure date.*
*If you have continued exposure (e.g. family member in the same home with COVID-19), then the 14 days doesn’t start until the COVID positive person is no longer contagious (i.e. has finished their quarantine period).
If COVID test is positive, then quarantine for at least 10 days since onset of symptoms AND at least 24 hours of improvement in symptoms.If COVID test is negative**, then it is okay to return to work/school/etc once you are fever free and improving for at least 24-48 hours.
**If we have high suspicion of a false negative test then you should treat as a positive test and continue to quarantine per positive test recommendations.
If COVID test is positive, then quarantine for 10 days from positive test date.
If COVID test is negative, then quarantine for 14 days from last known COVID exposure date.
